If you are a PHP , Inpatient, Residential or other Provider, and scheduling on behalf of a client, Please make sure the parent and or guardian of a minor or the adult client is aware of the appointment prior to scheduling. Please contact the provider you are scheduling under to verify the client will be coming from your program for safety reasons.
