Session Fee

It is $250 per session (50+ minutes per session) either in person or telehealth/video conference session payable at the time of service. – We do accept insurance, however, also offer a discounted cash pay option and have sliding scale fee for those with proof of financial need if choosing to not use insurance benefits – see below for more detail. We can discuss financial need during the consultation call or feel free to message me with any questions.

Do you accept Health Insurance?

Yes, however every plan is unique to the employer and or employee so while samantha is IN network with many plans, the Insurance determines which plans are covered. Some clients whos insurances are Out of network may have the option to opt out and switch to private pay cost and or use Out of Network benefits, read here for more information regarding pros and cons of not using insurance –

Samantha is independently contracted with:

Samantha has a new third party biller, and through virtual platforms Is accepting some EAP benefits

  • Tava Health : Tava is LIVE – has officially finished credentialing Samantha on many insurances – both national and regional plans, and private pay if preferred. – many insurances, including Anthem BCBS, Cigna, Aetna Health partners and more
  • Due to continued rising copays and deductibles – Samantha has chosen to return to Teladoc and Talkspace which do accept and offer discounts such as 0$ copays and or EAP sessions for many plans

We do not accept MedicareWe have no plans to start submitting claims / billing services / become in network- or – opt out of – Medicare at this time due to the platforms Samantha is connected to. Please contact your insurance directly for in network Medicare accepting provider.

Secondary Insurance

Occasionally plans will have secondary coverage, to cover what the primary insurance may not. This may be called an “HMO”. Please note -Samantha is not paneled with all insurance companies, therefore if your plan has a secondary insurance attached, Samantha may be considered “out of network” – please contact your insurance to ask about “OON- Out of Network benefits” for coverage for sessions, as they will only talk to the client and not the provider. You will also need to contact both insurances to ensure a “coordination of benefits” is in place prior to starting sessions to assist with easier billing if choosing to use your insurance benefits.

Why would I not use my insurance benefits?

For more information on choosing not to use insurance click here. Using your insurance is a personal choice however it can be confusing to understand and navigate. When using insurance for therapy, I am required to provide a diagnosis to the insurance company for reimbursement of services. Not everyone is comfortable with this option. Keep in mind if you choose to “opt out” of insurance first we must verify your insurance allows it and know that the fee will not go toward your deductible and cannot be refunded at a later time if choosing to “opt in”.

Does Samantha do her own billing or use a third-party billing service when communicating with insurance?
  • Starting June 2025 Samantha will be using a third-party billing service called “Tava Health” for clients using “Some but not all plans through : Aetna, Cigna, Blue Cross Blue Sheild (of Massachusetts) to start assisting with restructuring the insurance panels she is in network with.
  • Samantha has a platform to do her own billing with most major insurances, however some insurances have been known to be difficult in the past causing problems to the extent of Samantha preparing to remove contracts and no longer accepting referrals – Samantha will be utilizing Tava Health to assist with some of these insurances, as they will do the communication on behalf of both Samantha and the client at no extra cost to either side.
  • We have no plans to start submitting claims / billing services / become in network- or – opt out of – Medicare at this time due to the platforms Samantha is connected to. Please check your insurance portals directory for in network Medicare accepting provider.
  • We are Now out of network with Blue Cross Blue Shield /Anthem
Phone Consultation

We offer 15-20-minute complimentary video consultation to discuss concerns, and if we feel we are a good fit we can move forward with services or discuss a referral out.

Forms of Payment accepted

We accept cash, credit, Stripe or PayPal for your copay/coinsurance charge. We will send you electronic paperwork to add your insurance and card information at the time of the phone consultation if deciding to move forward with services at that time.  A card is required to be on file and paperwork must be filled out prior to the first intake session.

Fee Schedule
  • Please note your Copay if you have one is due at time of services. Cards will be charged at midnight day of session or by the end of the work week unless a private discussion is had regarding payment.
  • When using insurance, a claim is submitted at the end of the day and can take up to 45 days for Samantha to hear back, if there are any concerns or changes with copays or coinsurance at the time Samantha receives the acceptance or denial, Samantha will reach out to you.
  • Services may be covered in full or in part by your health insurance or employee benefit plan. If your insurance does not cover the full session they may state you have to pay a certain amount, known as copay or coinsurance. Please know that your rate will be based on what your insurance companies reimburse, which Samantha has no control over.
  • in the event you stop communicating and the card on file is unable to be charged, Samantha will submit your invoices and overdue charge account to a local debt collector company.
If I do not have insurance, or choose not to use it, do you accept cash payment?

Yes, many choose to have a flat rate discounted cash fee of $100 per 50-minute or 30 minute session, if choosing to have more then one hour the price will increase $50 per added hour. Cards are charged at midnight day of session. I currently accept cash, credit card, HSA card/check, or Pay Pal. With proof of financial need, we can discuss a lower rate. 

What is your cancellation policy?

Please let me know at least 24-48 hours in advance if you are unable to attend your scheduled time. There is a nominal charge of $50 for late cancelled appointments and no-shows, however, please be aware multiple late cancels/no-shows may end in termination of services.

NSF/Bounced Checks?

Will be assessed a $25 NSF (non-sufficient funds) check fee.

Questions To Ask Your Insurance Provider Before Your First Session:
  • Does my plan have out-of-network benefits?
  • Do I have a deductible? How much is it? Have I met any portion of my deductible with other medical expenses this year?
  • What is the cost or % of each session? (usually referred to as “co-insurance” payment)
  • Is there a limit for the number of “outpatient” mental health or psychotherapy sessions I am entitled to for the benefit year?
  • Does my plan require “pre-authorization” or a referral from a Medical Doctor (MD) or Primary Care Physician?
  • What amount or % of the psychotherapy fee will be reimbursed?
  • We will also work with you if we are not an “in-network” provider. For insurances we are not credentialed with, my services are considered “out-of-network” benefits and may ask for a “superbill”

Use this script when calling: Hello [insert insurance company] agency worker, my name is [state your name]. I am currently engaging in counseling services in the state of Wisconsin and would like to verify that my plan allows telehealth or video counseling. The billing codes 90834 or 90837 will be used – and my therapist has set up a secure video platform to see me. If they confirm that they allow this type of session, please ask for what is called a “Modifier” – usually a 2-digit code like: GT or 96. As we move forward this platform (video counseling) is available to nearly everyone who is working with me, and I am confident that it is only temporary, although it can be a valuable tool for us to connect, we work through your unique struggles.
